A student registration form is one of the required documents that the school uses during the registration process every academic year or every term to collect important and updated information about students and formalize their registration.  

The information collected in the form can help ensure student safety (asking for things like emergency contact information), match them to appropriate programs and student housing options (by uncovering preferences), and help manage student records in general by having all the information collected in one centralized place.

What should I ask in a student registration form? 

The type of questions that you ask during the student registration process will depend on the educational institution you’re building this form for. Identification and background questions 

These questions are common questions you see on all types of forms to help identify the student. Some examples:

  • First and last name
  • Home address
  • Mailing address 

Emergency contact information 

Asking the student, parent, or guardian for information on the best person to contact in the event of an emergency is critical on this form. Consider including these questions:

  • List the first and last name of your emergency contact and your relationship to them (repeat this 3x on the form)
  • Please indicate who you would like to be contacted first in the event of an emergency

Health-related questions

These questions help give the school a background of the student when it comes to their health and will help identify any support services the student might need.

  • List any current medications taken.
  • Personal health number
  • Health insurance provider and member number
  • Dental insurance provider and member number

Questions about program and housing preferences (colleges)

If your college or university has limited spots for its programs and student housing, getting the form respondent to select their top choices will help you match them accordingly. Be sure to ask for: 

  • First choice of program or faculty
    - arts
    - business
    - science
    - forestry
    - engineering
    - other
  • Second choice of program
  • Dorm preferences
    - single dorm

    - mixed gender building

  • Please list any specific requests or preferences around roommates (e.g. - same-sex or same culture)
  • Please list any physical disabilities or medical conditions that require specific accommodations (e.g. - a need to be on the first floor)
  • Length of time you plan to stay in the dorm (one semester / full academic year) 

Questions about living habits (colleges)

To help match students to living arrangements that suit them well, here are a few specific questions you can ask:

  • Do you consider yourself to be a morning person or a night owl?
  • How sensitive are you to noise in your environment?
    - prefer quiet
    - ok with some noise
    - enjoy a lively atmosphere
  • How would you describe your cleanliness and organization habits?
    - very neat
    - moderately organized
    - messy

Questions related to student finances 

These questions help the school identify whether it needs to confirm information from finance departments.

  • Have you received a scholarship from the school for the upcoming year or trm? If so, share details.
  • Will you be receiving any financial aid to fund your education? If so, share details.

Extracurricular activities 

This part of the form is where the student, parent, or guardian can list any extracurricular activities the student intends to be involved in or has already been involved in in the past.

  • Please select any extracurricular activities you intend to partake in
    (varsity sports, Model United Nations, student council, drama club, media club, other)

Document uploads 

It’s also worthwhile to ask the form respondent to upload any documents that the school might need to keep on record, including:

  • Proof of scholarship 
  • Doctor’s notes 
  • Updated study permit 
  • Previous year transcripts
